Greetings from Texas where Spring is trying to arrive. We will have several beautiful, sunny, warm days, then a cold front will drag temperatures back down into the 30s, followed by a day or two of rain. The good news is that our lakes are 99% full! Quite a feat considering the horrendous drought we had last summer. And, we are supposed to have an outstanding show of wildflowers in the next few weeks!

In this month’s newsletter, the lead article by Dr. Kendall-Tackett describes several recent articles about breastfeeding and nighttime sleep. Did you know that bedsharing, breastfeeding mothers get the most sleep when compared to breastfeeding, non-bedsharing mothers and formula-feeding mothers? And the sleep they get is better quality sleep!

Thomas W Hale Dear Dr. Hale,

Anything new on Agrylin & its safety with breastfeeding a newborn? I have a patient, 2 days postpartum with a diagnosis of essential thrombocytosis & chronic anemia. She has been on Agrylin 1.5 mg bid her entire pregnancy & hopes to reduce her dosage after talking with her hematologist. No other anti-coagulants other than occasional low dose aspirin. Her plan at present is to continue breastfeeding & have her infant checked periodically by her pediatrician. Anything new to tell her? She has the info from your 2008 MMM.

Sorry, no new data that I know of on Agrylin (Agarelide). I imagine the risk is low, but have no way to really support this. I think continuing breastfeeding with close followup by pediatrician is a good logical choice.

Academy of Breastfeeding Medicine – Released a press release on the Consumer Product Safety Commision’s (CPSC) warning on slings, stating that CPSC had missed the mark. ABM stated that the information in the warning was vague and misleading. Arthur Eidelman, MD, vice president of ABM stated, “All slings are not created equal. Unfortunately, however, CPSC issued a blanket warning about all types of sling carriers.” Ring slings protect the infant’s airway because the infant is carried in an upright position, snug against the parent’s chest. The unsafe slings referred to in the CPSC warning are the bag or duffle slings, where the infant may be curled up and the neck may be bent, making breathing more difficult.

Nighttime Breastfeeding and Maternal Mental Health

There is a movement afoot in childbirth education and perinatal health urging mothers to avoid nighttime breastfeeding to decrease their risk for postpartum depression. We know that if mothers follow this advice, it will have a negative impact on breastfeeding. But let’s put that issue aside for the moment and consider whether avoiding nighttime breastfeeding will preserve women’s mental health by allowing them to get more sleep. In short, is this good advice?
