Hale has adapted this manual to reflect the rapidly changing market. It has a useful introduction explaining each section to those who may not be familiar with physiochemical factors involved in evaluating drugs in lactation and determining the relative infant dose. Medications appear alphabetically by chemical name, accompanied by a list of trade names used in the United States, Canada, Australia, and the United Kingdom. For each drug, the pregnancy and lactation risk categories, adult and pediatric concerns, drug interactions, theoretical and relative infant doses, adult dose, drug alternatives, drug pharmacokinetics, and updated references are given. Clinicians may need to refer to other sources for specific information regarding mechanism of action, and pharmacodynamic and pharmacokinetic profiles. Radiocontrast agents, covered in a 1-page table in 2006, now occupy a 16-page appendix. Other additions include more than 80 new drugs and an appendix on birth contraception medications. Tables on radioisotopes, cough and cold medicines, and immunization schedule have been updated. Very disappointing, however, is the inclusion of two child development tables, unchanged from the 2006 edition, that are not referenced and that are “not derived from exclusively breastfed infants” (p. 1093-1094). Hale includes other substances that can affect lactation besides prescription medications such as over-the-counter medications, herbals, vaccines, blood modifiers, radioisotopes, insect repellents, breast implants, and vitamins. This manual includes how viruses like HSV, Epstein Barr, or Hepatitis A relates to breastfeeding and the risk to the child. With these inclusions, Hale has developed this functional reference guide to assist clinicians seeking alternatives to cessation of breastfeeding.
