Breastfeeding after Breast and Nipple Procedures: A Guide for Healthcare Professionals

Do you work with pregnant or breastfeeding mothers who have undergone breast or nipple procedures? Breastfeeding after these procedures can be challenging. Lactation consultant Diana West and (plastic surgery resident) Dr. Elliot Hirsch have teamed up to write Breastfeeding after Breast and
Nipple Procedures: A Guide for Healthcare Professionals.
West and Hirsch cover a variety of pertinent issues including:
• Predicting lactation capability after breast and nipple procedures
• Effects of tissue healing upon lactation capability
• Effects of diagnostic, ablative, and therapeutic procedures upon lactation functionality
• Breastfeeding during chemotherapy
• Effects of cosmetic breast surgeries on breastfeeding
• Lactation management in the post-breast or post-nipple surgical mother
• Supplementation to maximize breastfeeding
• Post-surgical complications after breastfeeding
• Effective breastfeeding counseling for mothers who have undergone breast or nipple procedures

For those who need reliable information on how various breast procedures and surgeries will affect breastfeeding in the mother’s they work with, this book is an invaluable resource.

The plethora of procedures and their implications reviewed provide a rich information base to counsel women to make fully informed decisions. The empathy and pragmatism underlying the discussion of various approaches to breastfeeding management provide a model for the ways health care practitioners can empower their clients towards a more satisfying breastfeeding experience. The initial chapters on the physiological implications include concise descriptions of a comprehensive array of diagnostic, ablative, and therapeutic procedures as well as chemotherapy. The chapter on cosmetic procedures starts by debunking the stereotype that women who augment their breasts are narcissistic. Evidence-based examples of the emotional reasons that women select these procedures enrich the descriptions of the physiological implications. Breastfeeding management chapters cover general approaches rather than highly prescriptive guidelines. The authors correct the misassumption that milk production cannot be altered by offering practical suggestions to counter obstacles common to all mothers regardless of needs of breast and nipple procedures. The section on using supplementation via bottles to support breastfeeding is sufficiently detailed to allow an inexperienced practitioner to master the technique. Indeed, with minor modifications, this section could easily be adapted as a stand-alone handout. Supplementation at the breast and via finger feeding is described in terms of how to determine their appropriateness. However, an inexperienced practitioner may need additional information or guidance to assist mothers to use these tools effectively. Eminently portable, this guide is easy to tuck away in a lactation bag along with other key reference guides. Selective clinical photos such as the “nipple nudge” and “vasospasm squeeze” and a crisp anatomical diagram augment the concise wording. Furthermore, the recommended books and websites will complement this guide with more in depth information on breastfeeding management. Susan E Burger, MHS, PhD, IBCLC New York, New York USA..
